Good Care Doesn’t Happen by Accident — It Starts With a Plan
When a loved one needs ongoing home health care, the difference between good care and great care often comes down to communication. Who is responsible for what? When does each service happen? What does the patient prefer, and what do they absolutely not want? A written care plan answers all of these questions before they become problems.
This template is designed to be completed collaboratively — ideally with your City Choice care coordinator — so that everyone involved in the patient’s care is working from the same page. It covers five areas: basic patient and plan information, up to three specific care goals with assigned responsibility and a target date, a weekly schedule showing which services are provided on which days, patient preferences and special instructions, and a key contacts section with space for the primary caregiver, physician, City Choice coordinator, and emergency contact.
The care plan is a living document. It should be reviewed regularly and updated whenever the patient’s needs change. The template includes a plan review date field to help make that a habit.
