• Lead-capture forms. When you download a free tool (assessments, checklists, planners), we ask for your first name, last name, email, and optionally your phone number, city, and care interest.
• Contact forms. When you reach out about home health services, we collect any information you choose to share — name, contact details, your loved one's situation.
• Patient referrals. Physicians, discharge planners, and social workers submitting referral forms share clinical and demographic information about the patient being referred. This information is handled in accordance with HIPAA and the related Notice of Privacy Practices we provide on intake.
